I removed the product tag, how can I provide the part number and production/tag code?

If you removed the product tag from your product, please attach a copy of your proof of purchase and a photo of the cover with your claim. Where the form requests the item number and tag code please enter the word REMOVED. 

If we are unable to determine what product you have from your proof of purchase, we will request that you measure your item so we my determine the size.
